By signing the form you agree to:

​

• We agree to pay our child’s tuition for preschool in advance of each month from the date of enrollment and summer camp tuition in advance of each week booked.

​

• We agree to give 30 day notice in writing if we intend to withdraw our child from preschool for health reasons or family relocation only.  

​

•  If your child has been exposed to any contagious disease or has an unexplained rash you agree to keep your child home until cleared by a doctor. The Child’s Garden reserves the right to send your child home if they are unwell at arrival and or if they present with symptoms during the school or camp day.

​

• We agree to being truthful about administering fever reducing medications 24 hours in advance of school or camp. 

​

• We agree to pay a late fee of $1.00 per minute for late pick up without a phone call in non-emergency situations please be on time at pick up.  we will add charges 

​

• We play/learn outside everyday, please dress your child for the weather and apply sunscreen before school or camp. Your child needs a complete change of clothes to be kept at school or camp and outdoor shoes that won’t slip off and can get dirty and will protect toes during outdoor play. A small water proof back pack, rain boots and a rain jacket and water proof gloves . (We are going to get very messy and dirty as we play and learn outside please save good clothes for home!) a detailed outdoor gear list by season will be provided.

• We agree to allow, or we will not allow our child’s picture to be taken and used on the child’s garden website, instagram and or face book page.

• We give permission for our child to walk to Charnews Agricultural Center on Youngs Ave. in Southold, NY to tend the school garden ( please note our walking route doesn’t include crossing any streets).
  I as the parent or guardian of the minor child named here in, give permission for them to participate in the gardening activities at the Child’s Garden Preschool and at the Peconic Land Trust Agricultural Center at Charnews Farm located at 3005 Young’s Avenue, Southold, New York.I understand that participation by the minor child named herein in these daily activities involves physical activity and risks of physical injury, and I have made a voluntary choice for myself and the minor child named herein to accept and assume all risks of injury that might be associated with or result from this activity. I hereby give consent for emergency transportation or treatment on behalf of my child. I further certify that the minor child named herein is in good physical condition and has no medical or physical condition that would restrict his/her participation in this event. He or she is in good health and is up to date with immunizations.

• We give permission for our child to walk to The Southold Public Library when planned. (Please note this walking route includes crossing Travelers street and the train tracks near Agway).
